国籍を問わず日本語で医師や看護師との会話が問題なくご自身の状態を伝えられる方は、通訳の同行は不要ですがご不安な方は医療通訳の手配をご自身でお願いいたします。当院には外国語を話せるスタッフはおりますが、治療の特性上毎回対応できるとは限らないためご理解いただくようお願い申し上げます。
確実な医療の提供のため、翻訳機のご利用やご家族・ご友人の通訳ではなく医療通訳専門の方をお連れください。また、通訳はオペ室に同行する可能性があるため必ず女性の方でお願いいたします。ただし、旦那様(男性パートナー)が日本国籍の方で毎回の診察に同行いただける場合に限り専門通訳の同行は不要です。

通訳を介した診療は、説明に時間を要するため、所定の追加料金を頂戴します。詳細は初診時にご説明します。日本語を話すことが可能な場合でも、治療を的確かつ安全に進めていくにあたり、状況によっては女性医療通訳者の手配をお願いすることもございます。予めご了承ください。

1. 身分証明書※必須

日本の健康保険証を(もしくは保険証登録済みのマイナンバーカード)をお持ちの方はご持参ください。
日本の健康保険証をお持ちでない場合は、お名前・生年月日が確認できる写真つき身分証明書(パスポートや在留カードなど)をお持ちください。

2. 問診票※必須

問診票をダウンロードしてご記入いただき、受診当日に持参してください。
日本語での記載が難しい場合は、通訳の方が代理でご記入ください。

他院の紹介状がある場合は初診時にお持ちください。
外国語で書かれた紹介状の場合必ず日本語で翻訳したものも一緒にお持ちください。

4. 基礎体温表

付けているものがあればお持ちください。なくても問題ございません。

5. 検査結果

1年以内に受けた感染症検査(クラミジア・梅毒・B型、C型肝炎・HIV)の結果をお持ちでしたら検査結果用紙の原本をご持参ください。検査結果をお持ちいただく際は、以下のページの「採血項目チェックリスト」を印刷してご記入いただき併せてお持ちください。

【法律婚のご夫婦】

院内でお渡しする治療同意書と併せて下記書類の提出をお願いしております。
書類を取得されるまでにお時間がかかる場合もございますので事前にご用意していただくとスムーズに周期に入れます。

• 移植周期に入るまでにご用意ください。年1回の提出が必要です。

『事実婚カップルへの同意書』にそれぞれ直筆でサインしたものを初診当日ご提出いただきます。こちらは事前にダウンロードをしてご準備いただくことが可能です。

・治療開始時までに別途それぞれの「独身証明」（3ヶ月以内に発行された原本）が必要です。
※但し日本国籍の方は戸籍抄本をご用意ください。

・移植周期に入る場合、独身証明書もしくは戸籍抄本の提出が必要ですが、前回提出した日から1年以内の方は再度提出する必要はございません。

Regardless of nationality, if you can communicate with doctors and nurses in Japanese without difficulty and convey your condition, you do not need an interpreter. However, if you are concerned, please arrange for a medical interpreter yourself. While we have staff who can speak foreign languages, we cannot guarantee availability for every appointment due to the nature of our treatments.
For accurate medical care, please bring a professional medical interpreter, not a translation device or family/friends. Interpreters may accompany you to the operating room, so they must be female.
However, if your husband (male partner) is of Japanese nationality and can accompany you to every appointment, a professional interpreter is not required.
Interpreted consultations require additional time for explanations, so there will be an additional fee. Details will be provided at your first visit.
Even if you can speak Japanese, a female medical interpreter may be required depending on the situation to ensure accurate and safe treatment. Thank you for your understanding.

1. Identification (Required)

• If you have a Japanese health insurance card (or a My Number card with registered insurance information), please bring it.
• If you do not have a Japanese health insurance card, please bring a photo ID that confirms your name and date of birth (passport, residence card, etc.).

2. Medical Questionnaire (Required)

• Please download and fill out the medical questionnaire and bring it with you on the day of your appointment.
• If you have difficulty writing in Japanese, your interpreter can fill it out on your behalf.

3. Referral Letter

• If you have a referral letter from another hospital, please bring it to your first appointment.
• If the referral letter is written in a foreign language, please bring a Japanese translation with it.

4. Basal Body Temperature Char

• If you have one, please bring it. It is not mandatory.

5. Test Results

• If you have test results for infectious diseases (chlamydia, syphilis, hepatitis B, hepatitis C, HIV) from within the past year, please bring the original test result documents.

Please prepare these documents in advance as it may take time to obtain them, allowing for a smoother start to your cycle.

• Please prepare these documents by the start of the embryo transfer cycle. Submission is required once a year.

• Japanese spouse: Family register extract (original issued within 3 months)
• Foreign spouse: Residence certificate proving marriage or certificate issued by an embassy/consulate

• Please submit the "Consent Form for Common-law Couples" with both partners' handwritten signatures on the day of your first visit. This can be downloaded and prepared in advance.

• A separate "Certificate of Single Status" (original issued within 3 months) for each partner is required by the start of treatment.
  ※However, Japanese nationals should provide a family register extract.
